EMHA Health Screening Module

We have uploaded (as best we can) all players and guardian info into our Health Screen Module.

Instructions on how to use it can be found on the following page:

If your email was provided then you should have received an email from SportsHeadz (check your spam), and you can follow the directions from there.  When signing up, please use your name not the players name.  Your player(s) name should be attached to yours once you sign up.

I've also created a spreadsheet with contact info that we would like to use going forward when group the players again on Oct 15th.

Here is a link to the contact info we need moving forward.  Please go to the sheet to update and fill in the blanks as it relates to your son(s)/daughter(s).  Doing so will make getting the Health Screening for the Oct 19 - Nov 29 time period much easier to implement quickly.


If you have not received an email from SportsHeadz, check the link above to see if your email was included.  If not please reach out to me and I can add it to the list.  If you email is on the document, but you still did not receive an email.  First check your spam / junk folder, and then contact me to let me know your situation.

I've also added a test practice for today for all teams.  You should be able to see it and test out it's functionality.
